I don't think handling every death separately is a good thing, since players can get confused about the penalty and when you suddenly get hard on them, they get pissed off. It can also lead to a sense of immortality when they realise that you could let them get away with dying.

Ofcourse it's harder to do this in CRPGs, there should be a 'fail'-option for DM, so he could make a monster attack really badly, either not hitting at all or making small damage. Ofcourse there is the DM healing and invulnerability-options, but I think they take away from the immersion. Suddenly gaining 100% resistance or getting magically healed is too easy. The difficulty bar works fairly well, but not well enough to make this simple.

Actually there is a function in NWN to deactivate an NPC or monster's AI on the fly. Doing so renders the NPC mindless, as if you took away its soul and stops fighting alltogether.

As for dealing differently with each PC, I don't think that's what we're saying. What I am saying is that a DM must deal with each situation as needed. PCs should all be in the same level range when they adventure together anyways. It's not like you'll have a level 1 PC playing with a group of 4 level 5. Dealing with a level 1 character differently than how you deal with someone level 4 or 5 is very understandable I think. And we're only talking about level 1 here. How long does a PC stay at level 1 anyway? A few minutes?

Different situations call for different solutions.

I like the idea of punishing people for their incompetence. If they are wandering ahead and trying to kill stuff without the party, or the guise of "being the scout," they should be penalized... particularly if another of the party dies because of monsters that were lured back.

I think respawn with severe XP loss is the best way to go. Permanent death is more PnP like....but I think it should really be a matter of DM's judgement.
