Question about NOTR
   

Do i have to start over to get to Jharkendar? (or whatever the name was)

Even if you don't want to go to Jharkendar you need to start over with the addon, because the old worlds have changed too and the learning system etc. You can't use any old save games with the addon, so you're obliged to start a new game.
