Hopefully, every Fire Beetle has a heart.
Hopefully, every Gargoyle has a skull.
Hopefully, every Dragon has Blood.
Hopefully, every Pixie has Dust.

The only way that I could even see as a logical solution to not getting the item (skull) after killing and looting a monster (gargoyle) would be if you inflicted too much damage on the creature to be able to harvest the reganent from it.

Correct.

Not correct (I believe that the white dragon did not leave me any blood).

Not sure, but I think this is correct. (I always felt bad killing pixies...)

I was kinda wondering why pixies were attacking me. I mean I am a good guy and here are these nice woodland pixies attacking them. I was like "don't have me tell the druids on you or you will be in big trouble"

The gargoyle skull is in a container in an office somewhere in the Snake Cult building....its a room close to the enterance where you talk to strange voice that lets you in.

Weird. The Gaoler (Jailer) never dropped ironwood for me. I have the platinum edition. Where else in the game can you get ironwood? Kinda wanted to make that club.
