Propylon Chamber Index's???

I have found a few chambers and when I clicked on them it said I need the right index or something. Now I came across something that said an index was in
Spoiler:
St. Olms Temple.
I've looked all over and can't find anything. Is it a book? Can they be purchased or do you I have to steal them? Is there a list of locations and how many you need?
Phew!!!! Thanks and I appreciate any help. I've been doing a lot of traveling and I read this is one way to make it easier.

They are teleport chambers and you need an index to activate them (one index for one chamber), try looking in a walkthrough if there is a list where you can find index stones.
