The ideas behind the games...

Here's a silly question:

Who's responsible for the ideas behind most computer software games? Is it the programmers, graphics designers, publishers, etc? Is it one person or a team of people? Is it everyone involved or aren't the storyline and ideas already put together before the game even begins being developed?

Here's another:

How do most game development teams get into game creation as a job? Do the majority aspire from the start of college and get a degree in software/game development specifically to make games? Do most coincidentally wind up in the game creation business? Or do most not even go to college and either get lucky or start from absolutely nothing but a serious addiction to games and everything related?

All thoughts, theories, and facts are greatly appreciated!

1. Everyone in the company contributes and brainstorms ideas.

2. Hardly anyone gets a "degree in game development". Most people go to college to learn computer science and mathematics. Then they either work their way up from tech support/entry level programmers or go to university for a few years so they can jump into their career higher up.
