Well I don’t know what new I’d like to see, but for certain I like to “upgrade” some competitions:
Sports like:
Archery, Shooting, or even Hammer throw - should have moving human targets
Fencing – sharper edges and race to 5 bloody cuts (bonus - a possibility of knock out)
Water sports – add some barracudas or sharks
Beach Volleyball – topless 
I guarantee you the tickets would be sold out weeks in advance

Axe trowing, normal one and de advanced (where they work in teams: 1 trows the axe to the teammate, who then trows it at the target.)
High diving without water would just be to cruel, the one that could still walk away would win.
What about tennis, badminton, tabletennis with exploding bals/shutles.

Split pegeons(they make it in a country)
And 2 teams with shotguns with real bullets try to kill each other.If their armor's good they survive if not..
They could have a dethmatch,cpture the flag,who kills with the most violented way...
