How do you Health, Magicka, and Fatigue?

What is the formula for calculating your Health, Magicka, and Fatigue based upon your Attributes? When I first started the game, my Health and Magicka both raised constantly each time I right-clicked to view my stats. They did this until they reached 100 and then stopped. Fatigue has increased as I have increased my Attributes each level, but Health and Magicka don't go up at all.

Unless you're a Breton or a High Elf, Magicka equals your Intelligence stat. Your starting health is, as I recall, half your combined health and endurance, and every level you gain something like a third of your endurance in health points.

I think.
