RPG Vault have posted Q&A #30 in their Dragon Empires feature. In this edition, Codemasters' Rob Adams talks about his role writing the game's lore. Here's a bit:
Jonric: What was the situation when you started? How much planning and actual writing had been done? What guidelines were in place, what overall direction were you originally given, and in what ways have these changed or expanded since then?

Rob Adams: When I began, there was a very basic outline of how the history of the game progressed with certain key concepts already composed. Other than this, there was little proper planning, and consequently, my task was to fill in the blanks and turn the skeleton of the lore into a fully fleshed entity.

The only writing existent when I began was the story by Piers Anthony, which remains the game's main inspiration. In the early months, I was provided with a pretty strong set of criteria to keep the lore in line with Piers' own vision. These guidelines remain in the production of all consequent lore.
How did I miss Piers Anthony's involvement till now?
