About the clothing of the hero:
The drawing is a little bit to clean here. Its not a T-shirt, actually a ragged tunic with the symbol of the divine. Remember he/she was sacrificed with lightning bolts. The triangles beside the lightning stand for praying/worshipping hands which means alltogether something like the cult/order/church of the divine.

Mostly the first, sometimes the second

As far as i know, yes.
