The mod changes the graphical appearance of items, increases the drop rate and values of rare and set items, it alters the size of the character stash/inventory and the Horadric Cube, new sounds are added and you'll encounter different monster classes (e.g. in the Chaos Sanctuary). There's no rating but the reviewer said if a new beta version would iron out the current bugs Hell on Earth could turn out to be very addictive.
