1Up.com has an interview with Lineage 2 lead designer, Raoul Kim. The conversation is presented in a preview-style article format; here's a taste:
Put the game first. Lineage II differentiates itself from most other MMOGs on the market by being a title that revolves entirely around combat. This combat is pervasive, but can be funneled down and explained by a single event: the castle siege. Predefined castles (five at present) dot the landscape of Lineage II's world. At timed intervals these castles can be attacked by opposing forces and defended by a single clan that lays ownership to the land.
