I know we all hate politics, but that doesn't mean we can't imagine a fun and attractive medieval politic system. At release time, all the leading positions in the world will be covered by NPCs, and maybe by some players from the beta test phase who achieved high leadership status. Also, all the laws will be set in an initial beginning state.
