Uvirith's Grave (Telvanni Stronghold) is indeed almost due west of Tel Fyr. There is a series of landmarks you can follow west in a more or less straight path. Right on the coast is the Sobitbael Renegade Ashlander camp, then Andas Ancestral Tomb, then Missir-Dadalit Egg Mine and Zaintirari Cave(these are north and south, respectively, of your westward path), then Zenarbael Smuggler's Cave, and finally your destination. If you reach Shushan, you've gone too far, that cave is southwest of Uvirith's Grave |
