Have you talked to him before?
Is this your first time talking to him?

Have you talk to Xardas yet? Did he give you a quest to save the Orc Shaman?

Do you go right? Or do you go left? A simple decision, yes? No ambiguities. Only the dream of the path not taken! And so one might stand in this place and contemplate for a long while. What will I miss if I go left? What will I lose if I go right? And in my indecision, have I chosen a worse fate?

You already have Uriziel? If that is so, then I think you don't need to talk to him anymore.

You are up to the sleepers temple again. Ur-Shak should be standing outside the orc town. You have nothing more to talk about. Go forth and vanquish the sleeper.

Ur Shak explains about the five shaman hearts and how you can get into the Orc Temple with the Ulu Mulu, which recipe you must get from his orc friend at the Free Mine.

It's only after all that and entering the temple that you find the Uriziel.

If it's your first time meeting Ur Shak then you shouldn't have the Uriziel yet (ahem...)

Where is Ur Shak exactly... ?
