I downloaded the demo a liked it, so I got the full version. I kept my savegames from the demo to use in the full game. When I loaded them, everything seemed fine, but then I noticed something weird: there were no NPCs or animals outside the demo area.

Then I found an apparently abandoned village. When I started find named weapons (Revenge Steel, Krush something-or-other) in chests and stuff, I knew something was wrong.

So my question is, do I have to start over?

The demo had only 1/3 of the population of the full game... you'll have to start over, no way around it.
